Company: Oak Street Health Title: Manager, Care Program Strategy & Operations (Vaccine) Location: Remote/Virtual Role Description: The Manager, Care Program Strategy & Operations (Vaccine) plays a critical role within Oak Street Health’s Population Health organization, supporting the design, implementation, and scaling of programs that improve patient health outcomes and advance value-based care objectives. This role focuses on vaccine strategy and execution, driving initiatives that improve immunization rates, close care gaps, and support preventative care delivery across the organization. Serving as a key cross-functional leader, this individual partners closely with Population Health, Clinical Operations, Nursing, Providers, Patient Safety, Epic, and external healthcare partners to develop and implement innovative solutions that improve patient outcomes and operational performance. The role requires strong project management capabilities, stakeholder influence, and the ability to execute complex initiatives across multiple teams and partners. This is a highly visible role responsible for bringing new ideas from concept to execution, including the development of pilot programs, evaluation of results, and scaling of successful interventions across Oak Street Health centers nationwide. The Manager will also support strategic partnerships and technical information exchange efforts to ensure accurate, timely, and actionable vaccine data is available to providers and care teams. Core Responsibilities:

• Support the execution, optimization, and long-term strategy of vaccine and preventive care programs across Oak Street Health.
• Partner with field-based clinical teams and regional leaders to implement efficient workflows that improve immunization rates and patient outcomes.
• Collaborate with cross-functional stakeholders including Nursing, Providers, Clinical Operations, Patient Safety, Population Health, and Epic teams to design and deploy scalable solutions.
• Lead initiatives that improve bidirectional information exchange between Oak Street Health systems, Immunization Information Systems (IIS), national networks, and external healthcare partners.
• Develop, launch, and evaluate pilot programs from concept through implementation, creating scalable operational models for successful interventions.
• Apply strong project management methodologies to drive execution of complex, multi-stakeholder initiatives and technical implementations.
• Establish and monitor key performance indicators related to vaccine administration, care gap closure, data integration quality, and clinical outcomes.
• Support healthcare partnerships and vendors to enhance program effectiveness and improve population health outcomes.
• Analyze clinical and operational data to identify opportunities, remove barriers, and recommend strategic improvements.
• Communicate program performance, insights, risks, and recommendations to leadership and key stakeholders.

What are we looking for?

• 5+ years of experience in healthcare operations, population health, consulting, clinical strategy, healthcare technology, or a related field.
• Bachelor’s degree required.
• Demonstrated experience leading complex cross-functional projects from planning through execution.
• Strong project management and organizational skills with the ability to manage multiple priorities simultaneously.
• Experience designing, implementing, and scaling operational or clinical programs.
• Exceptional stakeholder management skills with the ability to influence outcomes without direct authority.
• Experience supporting healthcare partnerships, vendor relationships, or external stakeholder engagement.
• Knowledge of healthcare interoperability, information exchange, clinical workflows, or healthcare technology solutions preferred.
• Experience with Epic and/or healthcare data systems preferred.
• Strong analytical and problem-solving skills with the ability to translate data into actionable operational strategies.
• Ability to thrive in fast-paced environments and navigate ambiguity with a proactive, solutions-oriented mindset.
• Excellent written, verbal, and presentation communication skills.

Pay Range

The typical pay range for this role is:

$66,330.00 - $145,860.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
